A: First print the form.

Then mail it (and your $8 check made out to the DMV) here:

Note: The Bill that brought these stickers into being is AB71. A list of the eligible vehicles.

And as an extra bonus:
There is a very nice lady named Gina at the ARB Motor Vehicle Info Hotline at 800-242-4450. She's the one to call if you see the stickers being abused. Get the license number, car model and if you can the sticker number and give this information to her.

Q: How do I obtain the California EV parking permit? (required for legally parking in a spot marked "EV ONLY" in CA)
A: Print and mail this form.
From the DMV Web site: "This form is used to apply for a Zero Emission Vehicle (ZEV) Parking Decal. A ZEV decal, when affixed to the rear driver side window or back bumper of an electric vehicle, authorizes the owner to park and recharge the vehicle in designated parking spaces." The cost of the decal is $17.

Official info for EV off-street parking which simply says this:
"ELECTRIC VEHICLES that are owned or leased by individuals or small businesses certified by the City's Emerging Small Business Development are eligible for free parking and recharging in designated parking facilities. Vehicles must be 100% electric, no hybrid vehicles are allowed. Call (916) 808-5110 for more information."
